There are three options for filing a claim – choose the one that’s most convenient for you and follow the instructions below. How to file by mail
Download and print out a claim form for your policy here.
Fill it out and attach copies of all required documents listed on the form.
Send the completed claim form to:

How to file from the employee portal
At this time the portal only accepts GAP/GAP+ and HIP/HIP+ claims. Claims for other Triada products need to be submitted via fax or mailed to the claims PO Box.
Log into your Triada Member Portal.
Once you’re logged in, select “View More” on the product you wish to file a claim for. From there, select the “File a Claim” tab to be directed to your claims portal.
Fill it out and upload attach copies of all required documents listed on the form and submit according to form directions.
